A 21st birthday is called the key birthday, or the key to the door, and unlike most birthday names this one has a real origin. In Britain and its former colonies, 21 was the age of majority until the late 1960s, and the custom was to hand the birthday person a key to the family's front door: they were now an adult who could come and go as they pleased. The key survives on cards and cakes long after the law moved the age of majority to 18. In the United States the 21st picked up a second meaning, the legal drinking age, which is why the American 21st is loud and the British 21st is sentimental.

Why the key
Before 1970 the age of majority in the UK was 21: the age at which a person could vote, sign contracts, marry without consent and be trusted with their own front door. The key was the symbol of all of it, and the song and the card followed. The law changed to 18 in 1970 and the symbol did not move with it, which is how traditions usually go. Australia, New Zealand and parts of Canada kept the same custom, which is why the 21st is a bigger deal in those countries than the 18th.
The American 21st
In the United States the age of majority is 18 but the drinking age is 21, so the 21st became the birthday of the first legal drink, with everything that implies. There is no gemstone for it and no borrowed anniversary name; it is simply the twenty-first. If you want a name, key birthday is the correct one and it travels.
